What is Popeye (Exophthalmia)?

Popeye (Exophthalmia) is a condition where one or both eyes protrude abnormally due to fluid accumulation behind the eye. It is most commonly caused by a bacterial infection (Aeromonas or Pseudomonas spp.), though injury or poor water quality can also trigger it. Unilateral (one eye) Popeye is often injury-related; bilateral (both eyes) typically indicates a systemic infection.

Symptoms

One or both eyes bulging outward from the socket
Cloudy or hazy eye surface
White ring around the eye in advanced cases
Lethargy and loss of appetite
Hiding behaviour
Possible blindness if untreated

Causes

Bacterial infection (Aeromonas, Pseudomonas) — most common cause
Physical injury — collision with decor or rough netting
Poor water quality — elevated ammonia or nitrite weakens immunity
Parasitic infection — internal parasites in some cases
Viral infection — less common

Popeye Life Cycle and Timeline

Day 1–3
Eye begins to protrude slightly and may appear cloudy. Fish may show mild lethargy or reduced appetite.
Day 4–7
Protrusion becomes more pronounced. Fluid accumulation increases. A white ring may appear around the eye. Fish shows visible discomfort.
Week 2
Advanced protrusion. Risk of eye rupture increases. Permanent vision loss becomes likely without treatment. Secondary infections may develop.
Week 3+
Without treatment, the eye may rupture or be lost entirely. Fish can survive with one eye but bilateral cases carry a significantly poorer prognosis.

Treatment Protocol

  • Quarantine the affected fish immediately to reduce stress and allow focused treatment
  • Perform a large water change and ensure pristine water quality in the quarantine tank
  • Add Epsom salt (1 tsp per 5 gallons) to help draw excess fluid from behind the eye
  • Treat with a broad-spectrum antibiotic targeting gram-negative bacteria
  • Test water parameters daily and maintain excellent water quality throughout treatment
  • Continue the full antibiotic course (7–10 days) even if improvement is seen early

Medication Options

  • Kanaplex (kanamycin) — highly effective for gram-negative bacterial Popeye; can be dosed in food
  • Furan-2 (nitrofurazone) — broad-spectrum antibiotic effective for external and internal infections
  • Maracyn-2 (minocycline) — effective for systemic bacterial infections
  • Epsom salt (magnesium sulfate) — 1 tsp per 5 gallons as an osmotic aid to reduce fluid behind the eye
  • Seachem Focus + MetroPlex — if a parasitic component is suspected alongside bacterial infection
  • Note: Even with successful treatment, the eye may not fully return to normal — some permanent protrusion may remain
Important

Popeye caused by injury often resolves with clean water and supportive care alone. Bacterial Popeye requires antibiotic treatment. Bilateral Popeye is a serious systemic infection — act quickly. Even after recovery, the eye may remain slightly enlarged permanently.
