What is Dropsy?
Dropsy is not a single disease but a clinical symptom of severe internal organ failure — most commonly kidney failure — usually caused by a bacterial infection (Aeromonas spp.). Fluid accumulates in the body cavity, causing the fish to bloat and scales to protrude outward in a distinctive pinecone pattern.
Symptoms
Severe bloating of the abdomen
Pinecone scales — scales protrude outward when viewed from above
Protruding eyes (Popeye)
Pale or reddened gills
Curved spine in advanced cases
Lethargy, loss of appetite, and bottom-sitting
Causes
Bacterial infection (Aeromonas spp.) — opportunistic in stressed fish
Poor water quality — high ammonia, nitrite, or nitrate
Weakened immune system from chronic stress
Viral or parasitic co-infections
Nutritional deficiencies over time
Dropsy Life Cycle and Timeline
Treatment Protocol
- Isolate the affected fish in a quarantine tank immediately to reduce stress and prevent potential spread
- Perform a large water change (50%) and ensure pristine water quality in the quarantine tank
- Add Epsom salt (1 teaspoon per 5 gallons) to help draw excess fluid from the body
- Treat with an antibiotic targeting gram-negative bacteria (Aeromonas)
- Maintain optimal water temperature for the species and ensure excellent aeration
- Begin treatment as early as possible — survival rates drop significantly once pineconing is advanced
Medication Options
- Kanaplex (kanamycin) — most effective antibiotic for internal Aeromonas infections
- Furan-2 (nitrofurazone) — broad-spectrum; can be combined with Kanaplex for severe cases
- Maracyn-2 (minocycline) — effective for systemic bacterial infections
- Epsom salt (magnesium sulfate) — 1 tsp per 5 gallons as an osmotic aid; reduces fluid retention
- Metronidazole — useful if a parasitic or anaerobic bacterial component is suspected
- Note: Dropsy is often fatal even with treatment. Early intervention gives the best chance of recovery
Dropsy is a symptom of severe internal failure and carries a poor prognosis, especially once pineconing is visible. Focus on prevention through excellent water quality and a nutritious diet. If a fish is suffering and unresponsive to treatment, humane euthanasia should be considered.
